The European Agency for Safety and Health at Work (EU-OSHA) highlighted the need for better work conditions for cleaning workers involved in nearly every industry. A verdict was pronounced against Thames Valley Police in a case where a civilian employee was injured seriously when a revolver believed to be loaded with dummy cartridges fired a live bullet instead.

The incident occurred at Oxfordshire where firearms instructor PC David Micklethwaite was conducting a training programme for new civilian recruits in TVP. Micklethwaite used ammunition from an unmarked sweet tin to load his revolver, thinking that the box had dummy cartridges. When he fired his weapon continuously to demonstrate the rotation of the cylinder, real shots were fired, one of hitting civilian staff Keith Tilbury in the stomach.

Mr Tilbury had to undergo treatment for serious bullet injuries in the Intensive Care Unit. He was also kept under observation at the hospital for a further period of about 10 days. The Judge hearing the proceedings remarked that Tilbury had suffered considerable disruption in his career owing to the injury. He pointed out that such gross negligence in mixing up live and non-live ammunition together was bound to result in such an accident.

HSE officials investigating the incident pointed out that risk assessment had in fact taken place, and the hazards of non-maintenance of ammunition in a systematic and easily accessible manner had been identified. Safety procedures and guidelines had also been determined and set down by TVP. However, these procedures were not brought into practice within the precincts, leading to the mix up. A call to employees to act upon risk assessment findings and implement safety measures was issued by the HSE following this judgement.
